A 64-year-old city man is facing an evading responsibility charge after Middletown Police say he hit a woman's car in a Washington Street plaza, left her a note with his personal information on it, and left the scene.

On Aug. 6 at 9:49 a.m., police responded to a minor hit and run accident at the Sunoco gas station on Washington Street, which the victim said had taken place in the Wow Fitness parking lot.

The victim said she just finished working at Wow at 9:30 a.m. and was heading home, when she saw a note under her windshield wiper, which read, "Please open, license plate [deleted] a white Chevy Silverado P/U 4X4 hit your vehicle," the report details. The right front of her car, the officer said, had slight damage.
